    Providing connection to multiple services for children, adolescents, and adults with or at risk of serious mental illness in their community to help each live the most fulfilling life possible. . A referral is required for enrollment.

    Program Overview

    At Wesley Family Services, our Enhanced Targeted Case Management approach is grounded in the belief that individuals—whether adults, children, or adolescents—living with or at risk of serious mental illness often benefit from the support of multiple services. Our mission is to help each person build the skills and capacity needed to live a full, meaningful life.

    Our services include:

    • Connection to essential resources

    • Individual advocacy

    • Mental health service planning

    • Ongoing monitoring

    • Building support networks

    • Assistance with resolving challenges

    • Promoting recovery and overall wellness

    The overarching goal is to support recovery, promote health, and reduce the need for hospitalization through a consistent, compassionate, and individualized approach.

    How Enhanced Targeted Case Management Helps

    Enhanced Targeted Case Management supports eligible individuals in accessing medical, social, educational, and other essential services. Since recovery often involves multiple supports, this service also helps to:

    • Coordinate and streamline services

    • Promote shared responsibility and collaboration

    • Support goal accountability

    • Identify resource gaps and barriers

    • Provide a structured process for resolving issues or conflicts
