Assertive Community Treatment (ACT/Westmoreland)
Providing psychiatric rehabilitation for adults 18+ with mental illness in home & community. A referral is required for enrollment.
The ACT teams provide 24/7 crisis services to those suffering from serious mental illness. These services include psychopharmacology, individual psychotherapy, supported employment, psychiatric rehabilitation, integrated dual-disorder treatment, peer support, and case management services. If additional care is necessary, we will refer clients to the appropriate source.
Our teams provide personalized attention and want to maintain our client’s independence, enhance the quality of life in social and employment settings, and provide support for families. Our teams consist of a psychiatrist, team leader, program assistant, four mental health professionals, a specialist in addiction counseling; three registered nurses; a vocational specialist; and a certified peer specialist.
Must have a serious and persistent mental illness
Must show difficulty using less restrictive services
Exhibit severe symptoms
Have high-risk factors
Be at imminent risk of homelessness
A referral through Westmoreland Case Management and Support Inc. (WCSI)
